Can somebody explain to me how to allow a user to select which fields they want included in a Query or report using a form on the Switchboard.

I can create queries and teach people to do that but I want something that shows field names on the form and if I select them they are included in a query.

Also if there is an additional field next to the selection boxes that they can enter criteria (like between dates or specific values for the fields they select).
